By using the alcohol monitoring services provided by Druchol Inc., the undersigned understands that Druchol Inc. does not guarantee the effectiveness, accuracy, or reliability of the monitoring process and disclaims any and all liability for any damages, losses, or injuries resulting from the monitoring or failure to monitor alcohol consumption.
The undersigned hereby waives, releases, and holds harmless Druchol Inc., its affiliates, employees, agents, and contractors from any and all claims, damages, or liabilities arising from the use of alcohol monitoring services.
Eligibility
